Unlimited Chiropractic Membership Rules & Guidelines
1. All Unlimited Chiropractic plan is designed to restore and maintain the spine and its nerve roots with maintenace care and is not for treatment of symptoms that resulted from recent auto accidents, work injury, or any other acute injury.
2. All memberships are based on monthly auto debit or annual prepaymenpayment by credit card or ATM debit card or you may prepay one year by any method.
3. All memberships are individual and nontransferable.
4. The entire immediate family can be added for an additional $20/mo on top.
5. Fees charged are a membership fee into the Unlimited Chiropractic Program which allows you to receive chiropractic adjustments as needed and are not for any particular number of treatments.
6. No cash or credit refunds for unused services.
7. Treatments are available during the regular hours of the office. During any vacation time that he doctor is not available there will not be any relief treament available and there will be no refunds given.
8. Ashley Chiropractic reserves the right to cancel clients membership at any time for any reason.
